The scenario illustrates the conversion of health care delivery system, UnityPoint Health- Fort Dodge, into an Accountable Care Organization (ACO) after the introduction of health reforms in US.  This scenario investigates in detail the way the hospital CEO and employees crafted and executed new versions of care delivery and staff designed and built relationships across health care delivery settings in an attempt to lower health care prices and better coordinate patient care.

Using patient narratives, care delivery before the creation of the ACO is compared to care delivery following the creation of the ACO. Three new plans that were bases to transformation attempts are emphasized: 1) decreasing re-entry 2) developing advanced medical groups and 3) creating alleviating program care.
