Patient safety is a key issue in health care remains a problem in almost all institutions of health care. The challenge is to change the culture so that all care providers put patient safety as a task. Traditionally, physicians have suggested that patient safety is due to a single failure. However, patient safety is a system and cultural change. The chief of the joint staff in a community hospital midsize wants to address patient safety, acting on his medical colleagues to change practice patient care without appearing condescending. The arena of patient care and changing attitudes is a new concept in Grand River Hospital. The case examines the various ways in which the Joint Chiefs can be used to introduce a culture of patient safety in the hospital.
